Fill out and sign our Rental Agreement. Please include 3 Trade References.

Step 2.

Send us a Copy of the Driver’s License of the responsible party.

Step 3.

We’ll need a valid Certificate of Insurance that includes:

• General Liability coverage of at least $1,000,000 USD as well as Miscellaneous Rented Equipment coverage for the value of the gear you are renting.
• “Worldwide” and “in transit” coverage.
• The following verbiage:
  “Certificate holder is named as Additional Insured and Loss Payee for Replacement Value as their interest may appear.”
• The Certificate Holder listed as:
  Rebel Camera LLC
  14811 NE 20th Ave
  North Miami, FL 33181

Step 4.

Complete our Credit Card Authorization Form and attach a copy of the front and back of the responsible party’s credit card.

Step 5.
